After filling out the application, please print out the direct service position form and fill it out, then print the criminal history affidavit and fill it out in front of a notary. These forms will be required when you come into the office for your interview.  

You may print out your I-9 form and tax forms and either fax them to 602-455-1107 or email them to cgoastaffing@gmail.com

Once you have had an interview with the office, you will be instructed to fill out the hire packet and dishonesty policy. Please wait until then before doing so.
